Intuitive Surgical Down 44%: Wall Street Eyes a Double-Your-Money Rebound
Published on: Jul 28, 2026

Intuitive Surgical (ISRG) has tumbled 44% from its peak, marking the stock’s second-steepest drawdown in a decade. Yet Wall Street isn’t walking away. According to CNN Business, 71% of analysts still rate the shares a buy, with a median 12-month price target of $485 — roughly 43% above the current level — and the most bullish targets imply a potential upside of 103%.

The sell-off was triggered by a demanding market fixated on slight deceleration. Second-quarter 2026 revenue rose nearly 19% to $2.89 billion, and adjusted earnings hit $2.80 per share, both beating consensus. But U.S. da Vinci procedure volume growth eased to 12% from 14% in the first quarter, and management flagged cost-containment risks in China, prompting short-term investors to head for the exits. Full-year procedure growth guidance was maintained at 13.5% to 15.5%, with adjusted gross margin expected at 68% to 69%.

A marginal slowdown in procedure growth has done little to dent the underlying franchise. The global da Vinci installed base now stands at 11,710 systems. The newly approved da Vinci 5 accounted for 52% of system placements in the quarter, lifting average selling prices. Hospital data shows da Vinci 5 delivers 11% higher utilization than previous generations, which translates into shorter procedure times, greater consumption of instruments and accessories, and deeper service ties.

Intuitive Surgical is, at its core, a razor-and-blade business. Systems made up only 23.6% of second-quarter revenue; more than 70% came from recurring streams — instruments, accessories, and services. Overall gross margin held above 66%. As the installed base expands and upgrades to da Vinci 5 accelerate, instruments and service revenue compound in layered, self-reinforcing fashion.

The valuation reset now provides the bull case with its strongest hook. Over the past decade the stock routinely traded at 64 to 65 times trailing earnings, and last year it peaked at 95 times. Following the pullback, the multiple has compressed to 38 to 39 times, sitting well below its historical norm. Analysts project earnings growth averaging 15% to 16% annually over the next three to five years.

Competitive dynamics remain benign. Intuitive still commands roughly 80% of the soft-tissue surgical robotics market. Deep-pocketed competitors have poured billions into developing alternative systems, but high switching costs and lengthy replacement cycles make rapid incumbency loss unlikely.

A steep sell-off has flushed out much of the stock’s valuation premium, while a recurring-revenue engine and the da Vinci 5 upgrade cycle continue to widen the competitive moat. Wall Street’s interpretation is unambiguous: price targets that project a double from these levels reflect a belief that the downturn is a long-term entry point, not a structural warning.
